I realize the important role I serve as an example to those in the United States and abroad. I understand that I represent not only my local church, but also Assemblies of God World Missions, and most importantly, Jesus Christ. In respect to this assignment, I will refrain from anything (e.g., alcohol, tobacco, unwholesome speech) that may distract from my Christian testimony, cause division, or demonstrate disrespect to the national church, missionary personnel, my team, or the Assemblies of God. I promise to forgo my personal convictions on these subjects in order to maintain unity and to avoid controversy in the body of Christ. I affirm that I do not have any criminal convictions or allegations related to sexual misconduct with an adult or minor, nor do I know of any reason I should not be allowed to work with adults or minors as a short-term missions volunteer.
Please answer the following questions honestly. These questions are not necessarily for the purpose of disqualification, but that the staff may understand where each applicant comes from and any major struggles in the lives of those wanting to join us on this journey. We fully believe in the restorative powers of God. The information you share with us will be treated confidentially. We may need to phone you if we have any questions.
If you currently have a passport and are applying for an international trip, please email a clear photo copy to allison@lifelinedenton.com with your name and desired trip in the subject line. If you do not have a passport, we will help you apply for one and will make a copy when you receive it.
I understand that as a participant of a Lifeline Church mission trip, I am voluntarily becoming an active member of a team. In doing so, I understand that all teams must have a team leader, and I will submit to the authority and leadership of the Lifeline pastors and team leaders overseeing this mission trip. I understand that my team leaders have the best interest of the team and myself in mind, and they will make decisions accordingly. They will do what is necessary at all times to protect the team and all members, while representing and sharing Christ among the people God has called us to serve. Therefore, I will positively represent Christ and Lifeline Church by following my team leaders’ instructions and directions at all times, and I will not do or say anything to undermine my leaders, the team, and especially the message of Jesus. If I have a concern or issue, I understand I may respectfully and privately bring it to the attention of my team leaders. I understand that being a part of a mission trip is a privilege and a high calling, and should I fail to comply with this commitment then I may be dismissed from the team and sent home early at my own expense.
I commit to full participation in Lifeline Church's mission trip to the location I have selected in this application during the designated dates. I understand it is my responsibility to raise my own funding for this trip, and I must raise the full amount set by the team leader and/or Lifeline staff. Should I fail to raise the designated amount for the trip and/or meet all payment deadlines listed below, I understand I may be removed from the team. Should I withdraw from the trip I understand I will be responsible for reimbursing Lifeline for any expenses they may have incurred on my behalf that are not already covered by the funds I have raised. I also understand that in all such cases all funds I have given to Lifeline for my chosen Expedition are non-refundable and will be used for missions trip funding by Lifeline.
